The Health Initiative’s flagship Community Health Clinic brings comprehensive, high-quality medical services directly to underserved communities. Led by Dr. Diana Nieves, MD (Chief Medical Officer) and supported by Dr. Josey Morey, MD (Board Member, Radiology Specialist), our clinic provides accessible primary care, preventive medicine, and diagnostic services to individuals and families regardless of their ability to pay.

Services Provided:

  • Primary care and comprehensive physical examinations
  • Chronic disease management (diabetes, hypertension, heart disease)
  • Preventive care and health screenings
  • Radiology and diagnostic imaging services
  • Women’s health and pediatric services
  • Senior care and geriatric assessments

Impact: Serving 500-1,000 patients in Year 1, with a sliding fee scale ensuring no one is turned away for inability to pay. Our patient-centered approach addresses not just physical health, but also connects patients to education resources and community opportunities.
