




Knowledge is power when it comes to health. The Health Initiative’s Health Literacy & Disease Prevention Program empowers underserved communities with the information and skills they need to take control of their health and prevent disease before it starts.
Core Programs:
- Health Literacy Workshops: Teaching how to navigate the healthcare system, understand medical terminology, communicate with providers, and read prescriptions
- Chronic Disease Prevention: Diabetes prevention, heart health, asthma management, and avoiding complications
- Nutrition on a Budget: Affordable healthy eating, meal planning, cooking demonstrations, and cultural food preparation
- Preventive Care Education: Understanding immunizations, cancer screenings, and routine health maintenance
Impact Goals: Year 1: 20-30 programs reaching 200-500 participants. Year 2: 60-80 programs reaching 1,000-1,500 participants. Year 3: 120+ programs reaching 3,000-5,000 participants.
