Campaign Overview
After a year of emergency response and temporary clinics, The Health Initiative set out to establish our first permanent Community Health Clinic. This campaign raised funds to secure a facility, purchase medical equipment, hire staff, and create a sustainable healthcare home for underserved communities.
Campaign Goal
Build a comprehensive primary care clinic offering medical services, education programs, and community health worker training—a permanent health home for families who had nowhere else to turn.
What the Clinic Provides
- Primary Medical Care: General medicine, chronic disease management, preventive care, women’s health, pediatrics, senior care
- Diagnostic Services: Lab work, X-rays, basic diagnostic testing
- Health Education: Wellness workshops, nutrition classes, disease prevention programs
- Mental Health Services: Counseling, stress management, support groups
- Patient Navigation: Help accessing specialists, medications, insurance
- Sliding Fee Scale: Affordable care based on income, no one turned away

Clinical Leadership
Led by Dr. Diana Nieves, MD (Chief Medical Officer) with specialty support from Dr. Josey Morey, MD (Radiology), our clinic brings decades of medical expertise to serve our community.
How Funds Were Used
- 40% – Facility lease, renovation, and setup
- 30% – Medical equipment and technology (EHR system, X-ray, lab equipment)
- 20% – First-year staffing costs (physicians, nurses, CHWs, admin staff)
- 10% – Pharmacy assistance fund and initial operating costs
Campaign Results: MAJOR SUCCESS
Goal: $150,000 | Raised: $175,300 (117% of goal)
Thanks to 620+ donors, major foundation grants, and corporate partnerships, we exceeded our goal and opened with additional capacity. The clinic has since served 3,500+ patients in its first year of operation.
